AAH Vouchers

Artists, arts administrators and their families may access free Artists’ Access to Healthcare (AAH) vouchers to be used, like a gift certificate, towards the cost of services at these low-cost clinics in the Twin Cities:

Everspring Health (alternative, Minneapolis): Two Free visits + 1 visit/month at a reduced rate of $25.
Family Dental Care (dental, Minneapolis): Two $40 vouchers.
N.I.P. Community Clinic (medical/dental/mental  health, Minneapolis): Two $40 vouchers.
People’s Center Health Services (medical/mental health, Minneapolis): Two $40 vouchers.
United Family Medicine (medical, St. Paul): Two $40 vouchers.

There are no income requirements. If you are uninsured or underinsured, you qualify! In addition to vouchers at each clinic, members of the Twin Cities Musicians Union (TCMU) get the added benefit of a $75 reimbursement for follow-up care at N.I.P. Community Clinic.

Here’s how to apply and use your vouchers:

➊ CLICK HERE to complete an online application (you may choose to apply for vouchers for one, two, three, four or all five clinics!).
➋ Applications are approved within 2 business days. The vouchers will be ready to pick up in our office or be mailed to you.
➌ Contact the clinic to make an appointment.
➍ Bring your voucher(s) with you to your appointment to receive the discount.

Voucher Terms: You may use one AAH voucher per visit, or two vouchers can be combined for up to $80 off during one visit. The full voucher amount must be redeemed (if you use less than $40, the remaining balance cannot be carried over).

Voucher Expirations: The first voucher for a free visit at Everspring Health expires one month from date of issue; the second voucher for a free visit and the $25 visits expire December 31, 2012. All other vouchers expire one year from their date of issue.

Reapply: You may apply for one Everspring Health voucher per calendar year. For all other clinics, you may apply for a new voucher once your current voucher has expired (apply once every 365 days).
